Getting Started

When referring a patient for a consultation at Gailey Eye Clinic, your staff can contact the Gailey Eye Clinic office of your choice to arrange an appointment with any of our physicians. Below are some things that help ensure a smooth transition.

There are two ways to schedule your patient for a consultation at Gailey Eye Clinic:

By Phone: Your staff may call any of our clinics to reach our scheduling staff.
By Fax: If you prefer our staff contact your patient directly to schedule an appointment, please fax clinical notes and indicate “call patient to schedule an appointment” on the cover sheet and we will contact them.

Information needed:

Exam notes
Patient insurance card(s)
Completed Gailey Eye Clinic Patient Consultation Form and/or patient demographic page

You may fax this information to any of our locations. It will become a part of the patient’s Gailey Eye Clinic electronic record for documentation purposes.
